What it is, How it works

Hair records a prolonged history of alcohol and drug presence by capturing biomarkers within the fibers of hair strands as they grow. When taken near the scalp, hair can offer an approximately three-month detection timeframe for alcohol and drugs. The collection process is straightforward, challenging to tamper with, and convenient for mailing.

A 1.5-inch piece consisting of roughly 200 hair strands (around the diameter of a #2 pencil) nearest the scalp will yield 100mg of hair, which is ideal for both screening and confirmation. For EtG tests, additives, and/or panels exceeding 10 in number, 150mg of hair is advised. It is recommended to use a jeweler’s scale for weighing the specimen. If scalp hair is unavailable, a similar quantity of body hair can be collected. The term "head hair" specifically refers to scalp hair, while "body hair" encompasses all other hair varieties, including facial and axillary.

Process Overview

The drug test laboratory procedure involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning focuses on the sample's entry into a lab’s system. This process verifies the sample's secure seal and proper shipment, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data input not handled by an electronic chain of custody system.

Screening represents an initial rapid assessment for drugs of potential abuse. Although Screening is an economical solution for excluding drug consumption in most samples, any positive screening results require confirmation for legal admissibility. Samples showing a presumptive positive in Screening need further confirmation.

In cases where a sample tests presumptively positive during Screening, additional hair from the initial collection is extracted and prepared for Extraction. At this stage, drugs are retrieved from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), hence making hair drug analysis the most complex methodology to execute.

The confirmation for any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing a presumptive positive result are cleansed prior to confirmation as necessary. The entire process from Accessioning through to Confirmation adheres to the CAP (College of American Pathologists) Hair designation and complies with the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
