If scalp hair is not accessible, a comparable quantity of body hair may be collected. The term head hair specifically refers to scalp hair alone, while body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample within a lab's system. This step includes confirming that the sample was appropriately sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplemental data entry not handled by an electronic chain of custody system.

Screening refers to an initial rapid assessment for substances of abuse. While Screening serves as a cost-efficient method to eliminate most drug use cases, a positive screen result must be validated for legal admissibility. All presumptive positive samples from Screening necessitate an additional confirmation step.

Upon obtaining a presumptive positive outcome during Screening, additional hair is extracted from the original sample and prepared for the Extraction phase. This step involves extracting drugs from hair at significantly lower concentrations compared to methods like urine or oral fluid analysis, making hair drug screening the most complex methodology to implement.

Confirmation of any positive screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples deemed presumptive positive are cleaned as required before confirmation. The entire process from Accessioning to Confirmation undergoes rigorous review under the College of American Pathologists (CAP) Hair criteria and complies with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
