What it is, How it works

Hair analysis has come to be recognized as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and other substances by embedding biomarkers within the fibers of the growing strand. When gathered near the scalp, hair can provide up to about a 3-month detection period for alcohol and other substances. Hair collection is straightforward, quite challenging to tamper with, and simple to transport.

A 1.5-inch segment of around 200 hairs (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg, which is the preferred amount for both screening and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's best to weigh the sample on a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair can be taken. "Head hair" refers strictly to scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial management of a sample within a lab’s framework. It involves ensuring that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances abuse. While Screening is an economical method to exclude drug usage in most samples, a positive result needs confirmation to be court-admissible. Samples that are presumptively positive in Screening must undergo secondary confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is selected from the original sample and prepared for Extraction. In this phase, substances are extracted from hair at substantially lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are cleansed before confirmation as needed. The complete lab procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
