A segment of hair measuring 1.5 inches, consisting of around 200 strands (comparable in size to a #2 pencil) closest to the skin provides 100mg of hair, which is the preferred quantity for initial testing and subsequent validation. For extended tests like EtG, additional panels beyond 10, a larger quantity of 150mg is advisable. Utilizing a jeweler's scale to measure the specimen is suggested. In cases where scalp hair isn't present, a proportionate amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. All other types of hair (such as facial and axillary) are classified as body hair.

Process Overview

The processing of a drug test result in the laboratory includes four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample within a lab's system. This process ensures the sample was correctly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and fills in extra details missing from the electronic custody record.

Screening is a preliminary rapid assessment for drugs of misuse. It is a cost-efficient strategy to generally rule out drug presence in samples, but a confirmed test is necessary for legal acceptance. Samples presumed positive at the Screening stage need reaffirmation.

Once a sample tests as presumptive positive in Screening, more hair is extracted from the initial specimen for the Extraction phase. Here, substances are removed from hair at lesser concentrations than in alternative methods (such as urine or saliva), thus making hair testing the most complex methodology.

Affirmation of any positive screen is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. Prior to affirmation, all samples that tested presumptively positive undergo washing. The entire lab protocol, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
