However, for drug use detection within the last 5 days, the urine test remains the most reliable.

What it is, How it works

Hair analysis is now regarded as an effective means to identify drug and alcohol consumption. It offers an extended account of substance use by capturing markers within the strands of hair as they grow. When taken near the scalp, hair can reveal up to about a 3-month period of drug and alcohol use. Collecting hair is straightforward, relatively tamper-resistant, and convenient to transport.

A sample measuring 1.5 inches and consisting of roughly 200 hair strands (comparable in thickness to a #2 pencil) directly from the scalp will yield 100mg of hair, which is the optimal sample size for analysis and validation. For EtG, additional tests, and/or examinations beyond 10 panels, 150mg is suggested. It is advisable to measure the sample on a jeweler's scale. If scalp hair cannot be obtained, a comparable quantity of body hair can be used. The term "head hair" specifically refers to scalp hair, while "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four major stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating a sample into a lab's system initially. This step includes ensuring the sample was sealed and dispatched properly, assigning a unique LAN (Laboratory Accessioning Number), and concluding any further data entry not captured by an electronic chain of custody framework.

Screening implies an initial rapid assessment for substance abuse. While it offers a budget-friendly method to exclude drug use in most samples, a positive Screening outcome must be verified for it to be legally viable. Samples that are initially positive during Screening must undergo secondary validation.

Should a sample yield a presumptive positive result at the Screening phase, more hair is extracted from the original specimen and readied for Extraction. During Extraction, substances are isolated from hair at substantially lower levels than in alternative methods (e.g., urine or oral fluids), highlighting why hair drug testing is especially complex to execute.

Authentication of any positive result from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are cleaned prior to verification as required. The complete laboratory journey from Accessioning through Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and accreditation conforming to ISO / IEC 17025 criteria.

Note: Although colloquially termed "hair follicle tests," the analysis targets the hair strand, not the hair follicle beneath the scalp.
