A sample of 1.5 inches comprising about 200 hair strands (approximately the thickness of a #2 pencil) close to the scalp will yield 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise measuring the sample on a jeweler's scale. If scalp hair isn't available, a similar quantity of body hair can be collected. When mentioning head hair, we mean only scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The primary four steps included in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a lab’s system. This consists of verifying that the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any further data entry not supplied by an electronic chain of custody system.

Screening entails an initial rapid test for drugs of abuse. While Screening is a cost-effective approach to negate drug use in most samples, a positive screen must be confirmed to be valid in legal settings. Any samples that are presumptively positive in Screening require a subsequent confirmation.

If a sample is assumed positive during the Screening level, additional hair is drawn from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at a noticeably reduced concentration compared to other methodologies (ex. urine or oral fluid), which is why hair drug testing is the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation when needed. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.
