The hair follicle test is prized for its extended detection timeline, capable of identifying drug use up to 90 days with monitored sample collection, ensuring accuracy and deterring tampering. It remains less suitable for identifying very recent use, typically within the last five days, where urine tests hold the advantage.

What it is, How it works

Hair analysis has emerged as a potent method for identifying drug and alcohol consumption. By capturing biomarkers within the fibers of the emerging hair strand, hair offers an extended history of alcohol and other substances. Collected near the skull, hair can offer a detection window extending up to around 3 months for alcohol and other substances. It's straightforward to collect, moderately hard to tamper with, and easy to transport.

A sample measuring 1.5 inches comprised of roughly 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for both screening and confirmation. For EtG, supplemental, and/or analyses exceeding 10 panels, 150mg of the sample is suggested. A jeweler’s scale is advised for weighing the specimen. If scalp hair is not available, an equivalent volume of body hair may be utilized. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the laboratory involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a specimen into the lab's framework. This process checks that the specimen was correctly sealed and sent, allocates a random LAN (Laboratory Accessioning Number), and finalizes any missing data entry not available via an electronic chain of custody system.

Screening conducts an initial rapid assessment for substances of abuse. Although Screening is an economical means to exclude drug usage in most specimens, a positive screen necessitates confirmation to be legitimate in legal proceedings. Any specimens deemed presumptively positive during Screening require an additional confirmation.

If a specimen shows presumptive positivity during the Screening phase, additional hair is taken from the original sample and prepared for Extraction. Here, drugs are extracted from hair at notably lower concentrations than in alternative techniques (e.g., urine or saliva), making hair drug analysis the most complex methodology to execute.

Confirmation of any positive screening outcome is executed utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo washing pre-confirmation as required. The comprehensive laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 criteria.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
