What it is, How it works

When collected near the scalp, hair can reveal substance use over roughly the past three months. The sample is easy to collect, relatively resistant to tampering, and simple to send for analysis.

A 1.5-inch portion consisting of around 200 strands (comparable to the width of a #2 pencil) from the scalp provides 100mg, the optimal amount for testing. For EtG, additional tests, or panels exceeding 10, 150mg are suggested. Utilizing a jeweler’s scale to weigh the sample is advised. If scalp hair isn't available, a comparable amount of body hair can be used. By head hair, we mean only scalp hair. The term body hair encompasses other hair types, like facial or axillary hair.

Process Overview

The laboratory drug testing process encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial acceptance of a sample into the lab’s system, ensuring it's been properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by digital tracking systems.

Screening provides a preliminary assessment for drugs. Though efficient for ruling out drug use in most cases, any positive result from Screening must undergo confirmation for legal validation. Presumptive positives detected during Screening require follow-up confirmation.

If Screening indicates potential drug use, additional hair is prepared from the original sample for Extraction. Here, drugs are extracted from hair at much lower concentrations than other methods (like urine or oral fluid testing), which is why hair testing poses unique challenges.

Confirmation of a positive screen uses GC/MS, GC/MS/MS, or LC/MS/MS techniques. All positive samples undergo a wash before confirmation if necessary. The entire sequence, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 certification.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
