For EtG, additional tests, and/or panels exceeding 10, a sample weight of 150mg is recommended. It's advisable to measure the specimen with a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair may be used. The term head hair specifically refers to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process that integrates a specimen into the lab’s system. It ensures the sample’s seal and shipment are intact, allocates a random LAN (Laboratory Accessioning Number), and completes any missing data entry not included in an electronic chain of custody.

Screening involves an initial rapid assessment for drugs of abuse. While Screening provides an economical method to eliminate drug presence in most samples, a positive result requires confirmation to stand in legal situations. Any sample that is flagged positively in Screening mandates further confirmation.

Should the Screening stage yield a presumptive positive, additional hair is extracted from the initial sample for the Extraction phase. Positive Screening confirmations employ GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptively positive undergo a pre-confirmation wash as needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
