What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. Hair holds a prolonged history of alcohol and other substances by capturing biomarkers within the strands' fibers as they grow. When collected near the scalp, hair can offer approximately a 3-month detection timeframe for drugs and alcohol. It is simple to collect hair, somewhat challenging to tamper with, and easy to transport.

A sample of 1.5 inches, containing roughly 200 hair strands (approximately the size of a #2 pencil) close to the scalp, provides 100mg of hair, the perfect quantity for analysis and confirmation. For EtG, additional tests, and/or panels exceeding 10, a sample of 150mg is advised. Using a jeweler’s scale is recommended for weighing the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair can be collected. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial process of a sample within a lab’s system. This step ensures the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid test for substance abuse. While Screening is a cost-efficient method to rule out drug intake in most samples, a positive result necessitates confirmation for legal admissibility. All samples presenting a presumptive positive in Screening require a secondary verification.

If during Screening a sample is found to be presumptively positive, additional hair is taken from the primary specimen and prepared for Extraction. At this stage, drugs are extracted from hair at much lower concentrations than through other techniques (e.g., urine or oral fluid), highlighting why hair drug testing is considered the most challenging to perform.

The Confirmation of any positive Screen result utilizes GC/MS, GC/MS/MS or LC/MS/MS methods. All presumptive positive samples are cleaned prior to Confirmation as necessary. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
