A sample of 1.5 inches, consisting of about 200 strands (comparable to the diameter of a #2 pencil) close to the scalp, yields 100mg, which is the optimal amount for both screening and confirmation. For EtG, supplementary tests, and/or panels exceeding ten, a specimen of 150mg is suggested. We advise using a jeweler’s scale for weighing the specimen. In the absence of scalp hair, an equivalent quantity of body hair can be used. "Head hair" pertains solely to scalp hair, whereas "body hair" encompasses all other hair types (face, underarm, etc.).

Process Overview

The process of laboratory drug test result processing comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning serves as the initial stage of integrating a sample into a laboratory’s workflow. This step ensures that the sample was properly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any additional information entry not covered by an electronic custody chain system.

Screening involves a preliminary assessment to detect drug substances. Although Screening is an economical method for dismissing drug usage across numerous samples, a positive screen necessitates confirmation to stand in legal proceedings. Samples indicating preliminary positive results during Screening require further confirmation.

Upon receiving a presumptive positive in Screening, additional hair is extracted from the original sample and prepared for the Extraction phase. Here, drug substances are isolated from hair at significantly lower concentrations compared to other techniques (e.g., urine or saliva), rendering hair drug analysis among the most challenging methods.

Confirmation for any positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Prior to confirmation of presumptive positive samples, washing is performed as necessary. The entire lab process from Accessioning to Confirmation undergoes review under CAP (College of American Pathologists) Hair designation alongside adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
