What it is, How it works

Hair testing is now perceived as a potent means for identifying drug and alcohol consumption. Hair retains a prolonged record of alcohol and other drugs by embedding biomarkers within the fibers of the hair as it grows. When collected near the scalp, hair offers a detection timeframe for alcohol and other drugs of up to roughly 3 months. Hair collection is straightforward, relatively difficult to tamper with, and easy to transport.

A 1.5-inch portion containing around 200 hair strands (roughly comparable to the width of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample on a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be taken. By head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the lab's system. This encompasses confirming that the sample was properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data input not supplied by an electronic chain of custody system.

Screening involves a preliminary swift assessment for drugs of abuse. While Screening is an economical method to exclude drug use in most samples, a positive screen must be confirmed for it to be court-admissible. Any samples that are tentatively positive in Screening require a secondary confirmation.

If a sample is provisionally positive at the Screening stage, additional hair is extracted from the initial sample for preparation in the Extraction phase. During this step, drugs are extracted from hair at much lower concentrations than other techniques (e.g., urine or oral fluid), which is why hair drug testing is the most challenging method to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed as needed before confirmation. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
