What it is, How it works

Hair testing is increasingly recognized as an effective method for discovering drug and alcohol usage. Hair captures a long-term record of alcohol and drug consumption by embedding biomarkers within the strands as they grow. If collected near the scalp, hair can offer a detection period of around 3 months for alcohol and drugs. Collecting hair is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch piece containing roughly 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample size for analysis and verification. For EtG, additional tests, and/or those beyond 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for measurement. Should scalp hair not be an option, a comparable volume of body hair can be gathered. When we mention head hair, we exclusively mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results generally includes these four key phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is first integrated into the lab’s system. This phase ensures the sample was properly sealed and delivered, allocates a random LAN (Laboratory Accessioning Number), and concludes any additional data input not incorporated by an electronic chain of custody system.

Screening performs an initial rapid assessment for drug abuse. While Screening is a cost-effective means to dismiss drug usage in most cases, a positive outcome needs confirmation for acceptance in legal contexts. Samples that show a presumptive positive during Screening mandate further confirmation.

If a Screening reveals a presumptive positive, more hair is extracted from the initial sample for the Extraction process. During Extraction, drugs are drawn from hair at a much lower concentration than other approaches (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Confirmation for any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair standards and compliance with ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
