What it is, How it works

Hair testing is now recognized as an effective method for detecting drug and alcohol consumption. This technique utilizes hair to record long-term use of alcohol and drugs by embedding markers within the strands as they grow. When procured near the scalp, hair can reveal substance use information for up to an approximate three-month span. Collecting hair is straightforward, challenging to tamper with, and convenient to mail.

A 1.5-inch segment consisting of around 200 hair strands (approximately comparable to a #2 pencil) closest to the scalp provides a sufficient 100mg sample suitable for screening and validation. For EtG, supplementary tests, and/or panels exceeding 10, it is suggested to obtain 150mg. It's advisable to measure the sample with a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be used. "Head hair" specifically means scalp hair, while "body hair" encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory preparation for a drug test result consists of four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage where a sample enters the lab's system. It involves confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data not covered by an electronic chain of custody.

Screening provides an initial rapid assessment for drug presence. Though cost-efficient for ruling out drug use in most samples, positive results in Screening require confirmation for legal admittance. Presumptive positives in Screening necessitate a confirmatory test.

For a positive indication during Screening, additional hair from the initial sample is used for Extraction, where drugs are isolated from hair at much lower levels than other tests (e.g., urine or oral fluid), making hair drug screening particularly complex.

Positive Screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to Confirmation as necessary. The entire assay procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair criteria and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
