What it is, How it works

Hair testing has gained recognition as a potent method for identifying drug and alcohol use. Hair holds a prolonged record of alcohol and other drugs by capturing biomarkers in the fibers of the growing hair shaft. When gathered near the scalp, hair can offer up to about a 3-month detection window for alcohol and other drugs. Hair is easy to gather, somewhat hard to tamper with, and convenient to transport.

A 1.5-inch segment consisting of around 200 strands of hair (roughly the size of a #2 pencil) closest to the scalp will yield 100mg of hair, the ideal sample for analysis and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair can be obtained. When referencing head hair, we mean scalp hair only. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The four primary steps involved in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a laboratory system. This includes ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary swift assessment for drugs of abuse. While Screening is a cost-efficient manner to rule out drug usage on the majority of samples, a positive screen needs validation to be allowed in court. Any samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is presumptively positive at the Screening phase, more hair is retrieved from the original sample and set for Extraction. In this phase, drugs are withdrawn from hair at a much lesser concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to conduct.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection window: Hair drug tests can spot drug use for up to 90 days, in contrast to urine tests with a shorter detection timeframe.
  • Difficulty to cheat: Cheating a hair drug test is very challenging, rendering the results more precise.
  • Provides historical data: It can deliver a pattern of drug use across a span of time, not merely recent use.

Limitations:

  • Cannot detect recent use: It requires approximately 5-7 days for drugs to become noticeable in hair.
  • Cost: Hair drug tests are typically more costly than other drug testing techniques.
  • Variations in results: Factors such as hair color and individual differences in hair growth can impact the concentration of drug metabolites in the hair.
