What it is, How it works

When clipped near the scalp, it can reveal up to a 3-month detection timeframe for various drugs and alcohol. Hair sample collection is straightforward, hard to tamper with, and easy to send.

A 1.5-inch sample consisting of roughly 200 hair strands (resembling a #2 pencil thickness) from the scalp procures 100mg of hair, optimal for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advisable. We advocate weighing the hair sample on a jeweler’s scale. If scalp hair isn’t available, a similar quantity of body hair is acceptable. Here, head hair refers only to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary processing of a sample into a lab’s system. It involves ensuring the proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and concluding any supplementary data entry absent from an electronic chain of custody system.

Screening includes a preliminary check for drug abuse. Although cost-efficient, a positive Screening result requires confirmation for court admissibility. Samples with presumptive positives in Screening necessitate additional confirmation.

Presumptive positives at the Screening phase lead to more hair being extracted from the original sample for Extraction. Drugs are extracted from the hair at significantly lower concentrations than other methods (such as urine or oral fluid), deeming hair drug testing the most challenging methodology.

Confirmation of any positive results from Screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as needed. The entire process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
