A sample of 1.5 inches, consisting of around 200 hair strands (comparable to the diameter of a #2 pencil) taken closest to the scalp will yield 100mg of hair, which is the perfect specimen amount for both screening and confirmation. For EtG, additional parameters, and/or tests exceeding 10 panels, the collection of 150mg is suggested. We advise using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, the same quantity of body hair might be collected. When mentioning head hair, we refer solely to scalp hair, while body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The major four stages in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of introducing a sample into the laboratory system. This process includes ensuring that the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing all supplementary data entries not provided by an electronic chain of custody system.

Screening performs an initial rapid detection for substance abuse. Although Screening is cost-efficient for excluding drug usage across most samples, any positive finding must be verified to be valid for legal proceedings. Samples that test presumptively positive in Screening require further confirmation.

If there is a presumptive positive result from Screening, additional hair is taken from the original sample for Extraction. During Extraction, drugs are removed from hair at much reduced concentrations compared to other methods (e.g., urine or oral fluid), illustrating why hair drug screening is the most challenging method to execute.

Confirmation of any positive Screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before confirmation. The comprehensive laboratory practice from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair accreditation and compliance with ISO / IEC 17025 standards.
