A 1.5-inch hair sample, consisting of roughly 200 strands (comparable to the thickness of a #2 pencil), equates to 100mg of hair—the optimal quantity for screening and confirmation. For EtG, additional tests, or panels exceeding 10, a 150mg specimen is urged. It is advisable to measure the sample using a jeweler’s scale. If scalp hair is inaccessible, an equivalent amount of body hair can be collected. When discussing head hair, it pertains solely to scalp hair. Body hair encompasses all other types such as facial and axillary hair.

Process Overview

The laboratory processing of drug test results comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating a sample into the laboratory system initially. This step includes verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry absent from an electronic chain of custody system.

Screening is a preliminary and efficient method for detecting drug abuse. Although it is a cost-effective strategy for discounting drug use in most cases, a positive screening must be confirmed for legal admissibility. Any presumptively positive samples from Screening require further confirmation.

Should a sample show a presumptive positive in Screening, a portion from the initial sample is prepared for Extraction. This phase extracts drugs from hair in much lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is among the most intricate methods to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation, as needed. From Accessioning to Confirmation, the complete laboratory process is evaluated under the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 standards.
