For EtG, add-ons, and/or tests beyond 10 panels, a specimen amount of 150mg is advisable. We suggest weighing the specimen on a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be taken. When mentioning head hair, we refer solely to scalp hair. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample into a laboratory's system. This step includes confirmation that the sample was properly sealed and transported, allocation of a random LAN (Laboratory Accessioning Number), and the completion of any extra data entry absent from an electronic chain of custody system.

Screening encompasses an initial rapid check for drugs of abuse. Although Screening is a cost-efficient method to exclude drug consumption in the majority of samples, a positive screen requires confirmation to be admissible in legal proceedings. Samples showing presumptive positive results in Screening necessitate secondary confirmation.

If a sample tests presumptively positive during the Screening phase, additional hair is obtained from the original specimen for Extraction. In this phase, drugs are pulled from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which accounts for the complexity of performing hair drug screening.

Confirmation of any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positive results undergo washing before confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
