For EtG, additional requests, or tests exceeding 10 panels, 150mg of the sample is advised. Utilizing a jeweler’s scale for measuring the sample is suggested. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. When mentioning head hair, it's specifically about scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary stages involved in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into the laboratory’s database. This step includes confirming the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening involves an initial, rapid assessment for drugs of abuse. Although Screening is a cost-effective means to eliminate drug use in most samples, a positive result must be validated for court acceptance. Any samples that are tentatively positive in Screening must undergo secondary confirmation.

When a sample tests positive in the Screening phase, additional hair is extracted from the original specimen for Extraction. In this phase, drugs are isolated from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), illustrating why hair drug analysis is the most challenging technique to administer.

Confirmation of any positive Screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and aligns with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
