A 1.5-inch hair sample consisting of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for assessment and validation. For EtG tests, additional analyses, and/or screenings beyond 10-panel tests, a quantity of 150mg from the sample is advised. It's suggested to measure the sample using a jeweler's scale. Should scalp hair be unavailable, a comparable amount of body hair can be collected. When referencing head hair, it specifically means hair on the scalp. Body hair encompasses other types of hair (such as facial or armpit hair).

Overview of the Process

The drug testing laboratory process comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is first processed into the lab's system. This stage includes checking that the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not covered by an electronic chain of custody.

Screening involves an initial rapid evaluation for signs of drug misuse. Although Screening is an economical method to dismiss drug use in most cases, a positive result requires a follow-up confirmation to be valid in legal settings. Any samples with a presumptive positive result from Screening need further confirmation.

If a sample is flagged as presumptive positive during the Screening, additional hair is utilized for the Extraction phase. Here, drug compounds are isolated from the hair at considerably lower concentrations than other methods (such as urine or saliva), justifying the complex nature of hair drug testing.

Confirmation of any presumptive positive outcomes from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples suggesting positive results undergo necessary washing prior to confirmation. The process from Accessioning to Confirmation is scrutinized following both CAP (College of American Pathologists) Hair guidelines and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
