What it is, How it works

Hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair captures a long-lasting record of alcohol and drugs by embedding biomarkers in the strands of growing hair. When taken from near the scalp, hair can present about a 3-month detection span for alcohol and drugs. Hair is easy to collect, relatively tough to tamper with, and simple to send.

A 1.5-inch segment of around 200 hair strands (about the thickness of a #2 pencil) from nearest the scalp will produce 100mg of hair, which is the best sample for analysis and confirmation. For EtG, add-ons, and/or screenings exceeding 10 panels, a 150mg specimen is advised. We suggest measuring the specimen on a jeweler’s scale. If scalp hair is not available, the same quantity of body hair can be obtained. When we discuss head hair, we mean scalp hair alone. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The key steps in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a lab’s system. This process encompasses confirming the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supported by an electronic chain of custody system.

Screening involves an initial brief assessment for drugs of misuse. While Screening offers an economical way to exclude drug use on most samples, a positive screen must be confirmed to be valid in court. Any samples that are tentatively positive in Screening must go through a secondary confirmation.

If a sample proves tentatively positive in the Screening phase, more hair is extracted from the original sample and made ready for Extraction. In this phase, drugs are drawn from hair at a much lesser concentration than in other methods (ex. urine or oral fluid), which is the reason hair drug screening ranks as the toughest method to carry out.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive specimens are cleaned prior to confirmation when necessary. The full laboratory procedure from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair designation and the authorization to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
