Process Overview

There are four key stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves logging a sample into a lab's system. This phase ensures the sample's seal and shipment comply with protocol, allocates a random LAN (Laboratory Accessioning Number), and completes any leftover data entry not managed by an electronic chain of custody.

Screening provides an initial rapid check for substance abuse. Although Screening is an economical way to eliminate drug use in most cases, a positive result necessitates confirmation to stand in court. Samples suspected to be positive in Screening require subsequent confirmation.

When a sample appears positive during Screening, additional hair is processed for Extraction. During this phase, substances are extracted from hair at a significantly reduced concentration than in alternative methods (e.g., urine or saliva), highlighting the complexity of hair drug screening.

Verification of any positive Screening findings occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples are cleansed before confirmation if needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
