What it is, How it works

Hair analysis is recognized as a robust method for uncovering drug and alcohol consumption. The strands of hair serve as a record of substance use over an extended period, capturing biomarkers within the hair as it grows. When taken near the scalp, hair can reveal a detection window of up to about three months for alcohol and other substances. The collection of hair is straightforward, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (similar to the diameter of a #2 pencil) at the closest point to the scalp will produce 100mg of hair, which is optimal for testing and verification. For EtG, supplementary tests, and/or testing beyond 10 panels, a total of 150mg is advised. We suggest using a jeweler’s scale to accurately weigh the sample. If scalp hair is not available, a corresponding volume of body hair can be collected. When we mention head hair, it specifically means scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The drug test process in the lab involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is where a sample is initially entered into a lab’s system. This step includes confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry that an electronic chain of custody system doesn’t cover.

During Screening, an initial, quick assessment for drugs of abuse is conducted. This step is an economical approach to discount the presence of drugs in most samples, though a positive screen must be substantiated for legal acceptability. Samples that test positive during Screening are required to undergo a secondary confirmation.

If a sample tests positive at the Screening stage, additional hair is extracted from the original specimen to prepare for Extraction. In this process, drugs are drawn from hair at concentrations significantly lower than those in other forms such as urine or oral fluid, which is why hair drug analysis is the most challenging methodology to execute.

Verification of any positive result from Screening is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Any samples that presumptively test positive undergo washing before confirmation as necessary. The whole laboratory operation from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
