If scalp hair is not present, an equivalent amount of body hair may be collected. References to head hair pertain solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory process for analyzing a drug test involves four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step, where a sample is entered into the laboratory's system. This involves confirming proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody.

Screening is a preliminary assessment to identify drugs of abuse. While Screening is an economical way to exclude drug use in most samples, positive results necessitate confirmation for court acceptability. Samples that are presumptively positive require further confirmation.

For samples that appear positive during Screening, additional hair from the initial sample is used for Extraction. This phase involves extracting drugs from the hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most complex methods to execute.

Confirmation of positive Screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as required before confirmation. The full laboratory process, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
