For EtG, enhancements, and/or tests exceeding ten panels, a 150mg sample is advised. Using a jeweler’s scale is suggested to weigh the sample. If scalp hair isn’t available, a comparable amount of body hair can be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair accounts for all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a sample within a lab’s system. This step includes checking that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and filling in any extra data entry absent in an electronic chain of custody system.

Screening offers an initial fast verification for drug abuse. Although Screening is economical and helps negate drug usage across most samples, a positive result requires confirmation for legal admissibility. Any samples that yield a presumptive positive result in Screening require a follow-up confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the original specimen for further Extraction. This stage involves retrieving drugs from hair at notably lower concentrations than alternative methods (e.g., urine or oral fluid), which makes hair drug screening one of the most intricate methods to execute.

Confirmation of any positive screening outcome is completed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation if necessary. The comprehensive lab process, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
