What it is, How it works

Hair analyses are now acknowledged as a potent method for identifying drug and alcohol consumption. Hair provides an extended historical profile of alcohol and other drugs by capturing biomarkers within the structure of the expanding hair strand. When taken near the scalp, hair can yield a detection window of up to roughly three months for alcohol and other drugs. It is straightforward to collect, relatively resistant to tampering, and simple to transport.

A 1.5-inch sample comprising approximately 200 strands (around the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional requirements, and/or tests surpassing 10 panels, 150mg is advised. We suggest weighing the sample on a jeweler's scale. If scalp hair cannot be procured, an equivalent amount of body hair can be collected. When mentioning head hair, it refers solely to scalp hair. Body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary integration of a sample into a laboratory’s system. It encompasses verifying that the sample was sealed and delivered correctly, allocating a unique LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening features an initial expeditious assessment for drugs of abuse. Although Screening is a cost-efficient technique to dismiss drug usage in the majority of samples, a positive screen requires confirmation to be admissible in legal proceedings. Any samples that indicate presumptive positivity in Screening necessitate secondary confirmation.

If a sample appears presumptively positive at the Screening phase, more hair is extracted from the initial specimen and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), which is a reason for the complexity of hair drug screening.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
