If a specimen tests presumptively positive during Screening, additional hair is extracted from the initial sample for preparation in Extraction. At this phase, drugs are withdrawn from hair in significantly lower concentrations than other methods (such as urine or oral fluid), highlighting why hair drug screening is one of the most challenging methods to perform.

Confirmation of any positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation if required. The complete lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and adherence to ISO / IEC 17025 standards.
