When collected from the scalp, hair can offer a detection span of up to approximately 90 days for alcohol and other drugs. Hair collection is straightforward, relatively hard to tamper with, and simple to transport.

A 1.5-inch section comprising around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal amount for screening and verification. For EtG, additional analyses, and/or tests beyond 10 panels, 150mg of the sample is advised. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be collected. When mentioning head hair, we specifically refer to scalp hair only. Body hair includes all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This step includes verifying the proper sealing and shipment of the sample, assigning it a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid check for drugs of abuse. While Screening provides a cost-effective means to exclude drug usage across most samples, a positive screen requires confirmation to be valid in court. Any samples that are apparently positive during Screening need secondary verification.

If a sample appears positive in the Screening phase, more hair is extracted from the original sample for Extraction. During this step, drugs are isolated from hair at a far lower concentration compared to other techniques (e.g., urine or oral fluid), which is why hair drug testing is among the most challenging methodologies to execute.

Verification of any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are cleaned prior to confirmation as needed. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
