Hair retains a prolonged timeline of substance usage by encapsulating biomarkers within the strands as they grow. When gathered near the scalp, hair can offer up to a roughly three-month detection span for alcohol and other drugs. It is straightforward to gather, relatively resistant to tampering, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (similar in size to a #2 pencil) nearest the scalp will produce 100mg of hair, the optimal sample for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advised. It's best to measure the sample using a jeweler’s scale. If scalp hair isn't accessible, an equivalent amount of body hair can be used. When we mention head hair, we mean scalp hair exclusively. Body hair refers to all other hair types, such as facial and axillary hair.

Process Overview

The primary stages involved in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into a lab's system. This step involves ensuring the sample was appropriately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing additional data input not provided by an electronic chain of custody system.

Screening acts as an initial rapid evaluation for substances of abuse. While it's a cost-effective approach to exclude drug use in most samples, a positive screen must be validated to be admissible legally. Any samples initially indicated as positive during Screening require further confirmation.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples are cleansed as necessary before confirmation. The entire laboratory cycle from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.
