For EtG, additional checks, and/or tests extending beyond 10 panels, 150mg of hair is advised. It is suggested to use a jeweler's scale to weigh the hair. If scalp hair isn't available, an equivalent volume of body hair may be used. In this context, head hair exclusively refers to scalp hair. Body hair encompasses other types such as facial and axillary hair.

Process Overview

The key phases in the lab processing of drug test results encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially inputting a sample into a lab's system. This phase ensures that the sample was correctly sealed and sent, a random LAN (Laboratory Accessioning Number) is allocated, and any further data entry not enhanced by an electronic chain of custody system is filled in.

Although Screening provides a cost-effective means to rule out drug use in most specimens, a positive screen must be confirmed to be legally admissible. Samples that are initially positive during Screening warrant additional confirmation.

Should a sample show a positive result during Screening, more hair is extracted from the existing sample and readied for Extraction. At this juncture, substances are extracted from hair at significantly lesser concentrations than other methods (e.g., urine or oral fluid), rendering hair drug testing the most complex method to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive specimens are cleansed before proceeding with confirmation, if necessary. The entire laboratory process from Accessioning through to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 criteria.
