For tests such as EtG, additional screenings, and those exceeding 10 panels, 150mg of the sample is advised. Utilizing a jeweler’s scale for weighing the sample is recommended. If scalp hair is not an option, an equivalent quantity of body hair can be utilized. By head hair, we denote scalp hair exclusively. Body hair pertains to all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of drug tests encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial integration of a sample into a lab’s database. This step ensures the sample is securely sealed and dispatched accurately, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data that an electronic chain of custody system does not provide.

Screening offers a preliminary assessment for the presence of drugs. Although it is a cost-efficient means to negate drug intake in most cases, any positive screening needs further confirmation for legal purposes. Samples that test positive in preliminary Screening require secondary validation.

If a sample returns a presumptive positive during Screening, additional hair is extracted from the sample and prepared for Extraction. Here, the drug compounds are isolated from hair in significantly lower concentrations than other testing methods (like urine or saliva), making hair drug testing one of the most complex methodologies.

Final Confirmation for a positive Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positives undergo washing if necessary. The entire laboratory workflow from Accessioning to Confirmation complies with CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
