What it is, How it works

A sample measuring 1.5 inches in length with approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, the perfect amount for initial screening and confirmation. For EtG, supplementary tests, and/or testing surpassing 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for precise weighing. If scalp hair isn't available, an equivalent quantity of body hair can be obtained. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (including facial and axillary hair).

Process Overview

The process of laboratory handling of a drug test consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial entry of a specimen into a lab's database. This process ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from the electronic chain of custody system.

Screening provides a preliminary, rapid assessment for substances of abuse. Although Screening offers a cost-effective means to negate drug usage in most cases, positive results require Confirmation to be valid in legal settings. Samples that are tentatively positive in Screening necessitate secondary confirmation.

Should a sample appear presumptively positive during Screening, extra hair is selected from the original sample and processed for Extraction. Here, drugs are extracted from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluids), explaining why hair drug testing is among the most challenging methodologies.

For any positive Screening results, Confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples that are tentatively positive undergo washing before Confirmation. The complete laboratory method, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and accreditation conforming to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
