What it is, How it works

A segment measuring 1.5 inches with about 200 hairs (approximately the girth of a #2 pencil) nearest to the scalp yields about 100mg, which is optimal for testing and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is advisable. It's suggested to weigh the sample using a jeweler’s scale. If no scalp hair is present, a comparable quantity of body hair can be gathered. When we mention head hair, we specifically mean scalp hair. Body hair includes any other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating the sample into a lab's system. This step confirms the sample is sealed and shipped correctly, assigns it a random LAN (Laboratory Accessioning Number), and completes any data input not covered by an electronic chain of custody.

Screening is a preliminary examination for substances of abuse. While it is economical for ruling out drug use in most samples, a positive screening must be verified for court admissibility. Any samples preliminarily positive in Screening necessitate further confirmation.

For samples preliminarily positive in the Screening phase, more hair is extracted from the original sample and prepped for Extraction. During this phase, substances are isolated from hair at much lower levels compared to other methods (such as urine or oral fluid), making hair drug analysis the most intricate method to conduct.

Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test preliminarily positive are cleansed before confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair standard and accreditation to ISO/IEC 17025 criteria.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
