Hair analysis has emerged as a robust method for identifying substance and alcohol consumption. Hair encapsulates a longitudinal record of alcohol and drug intake by capturing biomarkers within the strands as they grow. Sampling hair near the scalp offers a detection period of up to roughly three months for both alcohol and drugs. Hair collection is straightforward, relatively tamper-resistant, and convenient to transport.

A sample measuring 1.5 inches with around 200 strands (roughly the size of a #2 pencil) near the scalp typically provides 100mg of hair, which is ideal for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, a specimen of 150mg is suggested. It's advisable to measure the sample on a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be used. Head hair refers exclusively to scalp hair, while body hair encompasses other types (facial, axillary, etc.).

Process Overview

The drug test laboratory processing encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab system. This involves ensuring the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary additional data entry not included in an electronic chain of custody system.

Screening is a preliminary quick review for substances of abuse. While Screening is an economical method to eliminate drug usage for most samples, a positive result necessitates confirmation for legal admissibility. Samples presumed positive during Screening must undergo secondary confirmation.

If a sample returns a presumptive positive from Screening, extra hair is extracted from the specimen and readied for Extraction. During this phase, substances are extracted from hair at lower concentrations compared to alternative methods (e.g., urine or oral fluid), which makes hair drug testing more intricate.

Confirmation of any positive Screen results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as required. The complete laboratory process, spanning Accessioning to Confirmation, is reviewed against both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:
