Process Overview

The four fundamental steps included in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial categorization of a sample into a lab’s system. It involves confirming that the specimen was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening represents an initial swift check for drugs of abuse. Although Screening serves as a cost-efficient tool to exclude drug use on most samples, a positive screen necessitates confirmation to be acceptable in legal settings. Any specimens that are presumed positive during Screening require a secondary confirmation.

If a sample is assumed positive at the Screening phase, additional hair is retrieved from the primary specimen and prepped for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than with other methods (like urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening finding is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive specimens undergo washing before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 criteria.
