In cases where scalp hair is not available, a similar quantity of body hair can be gathered. The term head hair refers specifically to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the sample’s initial processing within a laboratory’s framework. This step involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry that might not be provided by an electronic chain of custody system.

Screening includes an initial swift evaluation for drugs of abuse. Although Screening is an economical method to exclude drug use on most samples, any positive screen must be confirmed to be legally permissible. Any samples initially deemed positive in Screening require a secondary confirmation.

If during the Screening phase a sample is initially found positive, more hair is taken from the original specimen and readied for Extraction. In this phase, drugs are removed from hair at significantly lower concentrations than other approaches (ex. urine or oral fluid), which makes hair drug screening a challenging methodology to conduct.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples initially positive are washed prior to confirmation as necessary. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
