What it is, How it works

When obtained near the scalp, hair samples can offer approximately a 3-month detection period for both alcohol and drugs. This method is straightforward to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp results in 100mg of hair, which is the optimal quantity for screening and verification. For EtG, additional tests, and/or assessments over 10 panels, 150mg of the sample is advised. We suggest measuring the sample using a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be gathered. References to head hair pertain solely to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

Accessioning entails the preliminary handling of a sample into a lab’s system. This involves confirming that the sample was securely sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not supplied by an electronic chain of custody system.

Screening executes an initial rapid check for drugs of abuse. While Screening offers a cost-efficient manner to discount drug usage in most samples, a positive screen needs confirmation to be acceptable in court. All samples with a presumptive positive result in Screening necessitate a secondary confirmation.

When a sample exhibits a presumptive positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In this step, substances are isolated from hair at much lower concentrations than other methods (such as urine or oral fluid), which is why hair drug screening ranks as one of the most challenging methodologies to conduct.

Verification of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples undergo washing before confirmation as required. The full lab process from Accessioning to Confirmation is scrutinized according to the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
