What it is, How it works

When the hair is taken near the scalp, it allows for a detection period of up to 3 months for alcohol and drugs. Hair collection is straightforward, quite hard to tamper with, and easy to transport.

A 1.5-inch piece of around 200 hair strands (similar to the thickness of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the perfect sample size for analysis and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. We recommend measuring the specimen with a jeweler’s scale. If scalp hair is not accessible, a similar quantity of body hair may be gathered. Mention of head hair pertains only to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for drug testing results encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary incorporation of a sample into a lab’s system. This encompasses ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data input not supplied by an electronic chain of custody system.

Screening involves an initial rapid assessment for drug presence. While Screening is a budget-friendly method to negate drug use in most samples, a positive screen demands confirmation for judicial acceptability. Any samples showing potential positive results in Screening necessitate secondary confirmation.

If a sample yields a potential positive result in the Screening phase, additional hair is extracted from the initial sample and set for Extraction. In this phase, drugs are isolated from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to perform.

Confirmation for any positive Screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with potential positive results undergo washing before confirmation as required. The complete laboratory process from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
