In cases where scalp hair is not available, equivalent quantities of body hair can be used. The term head hair refers exclusively to scalp hair, whereas body hair includes all other types (such as facial or underarm hair).

Process Overview

There are four primary stages in the laboratory analysis of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of entering a sample into the laboratory system, ensuring it was properly sealed and shipped, assigning it a random LAN (Laboratory Accessioning Number), and making any necessary data entries not supplied by an electronic chain of custody.

Screening is a swift initial evaluation for illegal substances. Although cost-effective, a positive result from Screening requires a follow-up to be legally valid. Samples that test preliminarily positive in Screening undergo additional confirmation.

If initial screening indicates positivity, extra hair is taken from the original sample and prepared for Extraction, where drugs are isolated at concentrations much lower than those in urine or oral fluid testing, highlighting the challenges of hair drug testing.

Verification of any positive findings from screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. Prior to confirmation, presumptive positive samples are cleansed as needed. The entire process, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair permissions and in line with ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
