When harvested near the scalp, hair can offer up to roughly a three-month detection period for alcohol and other substances. Hair is easy to obtain, relatively tough to tamper with, and straightforward to transport.

A 1.5-inch portion consisting of about 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for screening and verification. For EtG, supplementary tests, and/or panels exceeding 10 tests, 150mg of the sample is advisable. We advise using a jeweler’s scale to weigh the sample. If scalp hair is nonexistent, an equivalent quantity of body hair can be collected. When mentioning head hair, we reference exclusively scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the lab's system. This covers verifying the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any further data logging not accommodated by an electronic chain of custody system.

Screening includes an initial rapid assessment for drugs. While Screening provides a cost-efficient method to exclude drug use in the majority of samples, a positive screen must be confirmed to be permissible in legal settings. Samples showing presumptive positivity during Screening necessitate secondary validation.

If a sample is deemed presumptive positive in the Screening phase, additional hair is sourced from the initial sample and prepped for Extraction. In this phase, substances are extracted from hair at a significantly lower concentration than through other methods (e.g., urine or oral fluid), illustrating why hair drug analysis remains the most challenging method to conduct.

Confirming any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity undergo washing before confirmation as required. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
