A sample measuring 1.5 inches from about 200 hair strands (equal to a #2 pencil's thickness) nearest to the scalp yields 100mg, which is optimal for screening and confirmation. For EtG, additional tests, or panels exceeding 10, 150mg is advised. We suggest using a jeweler’s scale for precise weighing. Should scalp hair be unobtainable, an identical quantity of body hair can be used. When discussing head hair, we strictly mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The lab processing of drug tests involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a lab system, ensuring the sample's secure sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional required data entry not handled by an electronic chain of custody system.

Screening performs a preliminary check for substance abuse. While it's a cost-efficient way to eliminate drug use for most samples, a positive result must be confirmed for court acceptance. Samples that are initially positive during Screening need further validation.

Should a sample show a presumptive positive in Screening, more hair is extracted from the initial specimen for the Extraction process. Here, drug extraction occurs at a much lower concentration compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug analysis.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation if necessary. The full lab process, from Accessioning through Confirmation, undergoes review per the CAP (College of American Pathologists) Hair criteria and ISO/IEC 17025 certification standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
