For EtG, additional tests, and/or more than 10 panels, a 150mg example is advised. We recommend the sample be weighed on a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be collected. Head hair specifically refers to scalp hair, whereas body hair includes all other hair types, like facial or axillary.

Process Overview

There are four primary stages in the laboratory drug testing process: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into the laboratory’s system. This step checks that the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary examination for drug abuse. Although it’s a cost-effective way to identify drug use in a majority of specimens, any positive screen must be confirmed to hold up in court. Samples that test presumptively positive in Screening require a secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the sample for Extraction. This step retrieves drugs from the hair at a lower concentration than other methods (like urine or oral fluid), which makes hair drug screening the most challenging method to perform.

Confirmation for any positive screening result is performed with GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
