A 1.5-inch section of approximately 200 strands of hair (about the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the preferred sample for screening and confirmation. For EtG, added tests, or tests beyond 10 panels, 150mg of hair specimen is suggested. It's advisable to weigh the collection on a jeweler’s scale. If scalp hair isn't available, an equivalent quantity of body hair may be gathered. "Head hair" specifically refers to scalp hair, while "body hair" covers all other types (such as facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within a lab's system. This step includes confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.

Screening entails a preliminary quick assessment for drug substances. Although Screening is a cost-efficient method to eliminate drug usage in most samples, a positive result must be confirmed for court admissibility. Samples that are preliminarily positive in Screening need a secondary validation.

If a sample turns out presumptively positive during Screening, extra hair is pulled from the main sample and prepared for Extraction. In this phase, drugs are extracted from hair at concentrations significantly lower than in other methods (e.g., urine or oral fluid), which is why hair drug testing is among the more challenging methods to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is carried out in accordance with both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
