What it is, How it works

Hair screening is increasingly recognized as a compelling method to detect drug and alcohol consumption. By encapsulating biomarkers in the fibers of a growing hair strand, hair delivers an extensive history of alcohol and other drugs. When obtained near the scalp, hair offers a detection window of up to 3 months for alcohol and other drugs. The collection process for hair is straightforward, not easily tampered with, and convenient for shipping.

A sample measuring 1.5 inches, consisting of around 200 hair strands (roughly equivalent to a #2 pencil in diameter) taken closest to the scalp, yields 100mg of hair, the preferred quantity for analysis and verification. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the sample is advisable. We propose using a jeweler’s scale for weighing the specimen. If scalp hair is unavailable, a similar amount of body hair can be used. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other varieties of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test comprises four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial entry of a sample into a lab’s system, ensuring the sample was securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not handled by an electronic chain of custody system.

Screening provides an initial rapid check for drugs of misuse. While Screening is an economical method to eliminate drug usage in the majority of samples, a positive screen must be verified to be acceptable in court. Any samples that test presumptively positive in Screening must undergo a secondary confirmation.

If a sample results in a presumptive positive during the Screening stage, more hair is extracted from the original specimen and readied for Extraction. During this phase, drugs are pulled from hair at a much lower level than with other methodologies (such as urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The full laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
