A 1.5-inch segment of approximately 200 hair strands (comparable to the size of a #2 pencil) from closest to the scalp will provide 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or assessments beyond ten panels, a sample of 150mg is advised. We suggest utilizing a jeweler’s scale for weighing the specimen. If scalp hair is not available, a similar quantity of body hair can be used. In the context of head hair, we specifically mean scalp hair. Body hair refers to any other hair type (facial, axillary, etc.).

Process Overview

The four key steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into the lab’s system. This step includes ensuring that the sample has been sealed and shipped properly, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening consists of an initial rapid assessment for drug abuse. Although Screening is an economical way to dismiss drug use for most samples, a positive screen requires confirmation for courtroom admissibility. Any samples found presumptively positive during Screening necessitate a secondary confirmation.

If a sample is found presumptively positive during Screening, more hair is drawn from the initial specimen and readied for Extraction. During this phase, drugs are extracted from hair at a much lower concentration than in alternative methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screen result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as necessary. The complete lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
