If scalp hair is inaccessible, an equivalent quantity of body hair can be obtained. References to head hair pertain only to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

If a sample indicates presumptive positivity at the Screening stage, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than in alternative methodologies (ex. urine or oral fluid), explaining why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
