Nonetheless, urine tests remain best for detecting recent use within five days.

What it is, How it works

Hair analysis has emerged as a potent method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and drugs by embedding biomarkers within the fibers of the hair as it grows. When extracted near the scalp, hair can offer nearly a 3-month window for detecting alcohol and drugs. The collection of hair is straightforward, generally resistant to adulteration, and can be shipped easily.

A 1.5-inch portion consisting of around 200 hair strands (roughly the size of a #2 pencil) near the scalp will provide 100mg, which is the optimal sample for both screening and confirmation. For EtG enhancements or tests exceeding 10 panels, 150mg is suggested. Utilizing a jeweler's scale for weighing is advisable. If scalp hair cannot be obtained, an equivalent amount of body hair can be used. In discussing head hair, we mean scalp hair specifically. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a laboratory's system, ensuring the sample was sealed and sent correctly, assigning a random Laboratory Accessioning Number (LAN), and completing any data entry not covered by an electronic chain of custody.

Screening is a preliminary check for substances of abuse. Although Screening offers a cost-efficient approach to dismiss drug use in most samples, a positive result requires confirmation for court admissibility. Any presumptive positives in Screening need further confirmation.

If a sample is indicated as positive during the Screening, additional hair is extracted from the initial specimen for Extraction. In this phase, drugs are separated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), thereby making hair analysis one of the most challenging methods to conduct.

Final confirmation of any positive Screening outcomes is done using GC/MS, GC/MS/MS, or LC/MS/MS. Samples that return presumptive positives undergo washing prior to confirmation as necessary. The end-to-end laboratory process from Accessioning to Confirmation aligns with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
