A sample measuring 1.5 inches consisting of around 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal sample for both initial screening and subsequent confirmation. For EtG testing, additional tests, and/or panels exceeding ten, a sample weighing 150mg is preferred. It is advisable to measure the sample weight using a jeweler's scale. In the absence of scalp hair, an equivalent amount of body hair can be utilized. When mentioning head hair, it specifically denotes scalp hair. On the other hand, body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The process of laboratory assessment of a drug test outcome encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into a laboratory database. This step includes verifying that the sample was adequately sealed and dispatched, allocating a unique LAN (Laboratory Accessioning Number), and completing any additional data processing potentially absent in an electronic chain of custody system.

Screening serves as an initial rapid assessment for substances of abuse. Though Screening provides a cost-effective strategy to identify the absence of drug use in the majority of instances, a positive result necessitates confirmation for courtroom admissibility. Every sample that results in a presumptive positive during Screening requires further confirmation.

In instances where a sample yields a presumptive positive during the Screening phase, additional hair is retrieved from the primary sample for Extraction. This phase entails extracting drugs from hair at a significantly reduced concentration compared to methods such as urine or oral fluid testing, contributing to the higher complexity of hair drug screening.

Verification of any positive screening result is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All presumptively positive samples undergo a washing procedure before confirmation if necessitated. The entire laboratory workflow from Accessioning to Confirmation is scrutinized according to the CAP (College of American Pathologists) Hair designation along with the accreditation meeting ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
