In the absence of scalp hair, an equivalent amount of body hair can be used. "Head hair" implies only scalp hair, while "body hair" covers all other types of hair (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves processing a sample into a laboratory's system. This stage includes ensuring the sample was securely sealed and delivered, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody.

Screening provides an initial rapid assessment for drug presence. While Screening is cost-effective for dismissing drug usage in most samples, a positive result must be validated for court acceptance. Presumptive positives from Screening require further confirmation.

If a sample yields a presumptive positive in Screening, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are extracted from the hair at significantly lower concentrations than other methods (e.g., urine, oral fluid), which makes hair drug testing a more complex methodology.

Confirmation of positive screening results is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing as necessary before confirmation. The process from Accessioning to Confirmation complies with both CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
